Sangre de Cristo Community Care provides quality of life and dignity to those whom we extend compassionate care during end of life difficulties. It was established to serve all terminally ill patients and their families without regard to age, race, religion, color, sex, marital status, national origin, handicap, sexual orientation, veteran status, and regardless of the patients’ place of residence in the service area.

Home Health Care Services

Sangre de Cristo Community Care has been providing home health services to the communities of southern and eastern Colorado since 2018. In 2021, Sangre was named an HHCAHPS Honors Elite recipient by HEALTHCAREfirst. We are committed to continuing to provide excellent care in our communities in Trinidad, Walsenburg, Cañon City, Westcliffe, Wetmore, and La Junta.
